Oncology Pharmacist
Overview
This role offers an opportunity to apply your oncology pharmacy expertise in a clinical setting, ensuring safe and effective chemotherapy preparation and administration. You will collaborate closely with healthcare providers to optimize patient care and maintain compliance with regulatory standards. Key Responsibilities ● Assist Pharmacy Buyer in maintaining accurate patient chemotherapy agent lists and par levels to minimize waste ● Review pharmacy inventory to ensure adequate stock levels and remove outdated or unused drugs ● Promote use of formulary system and work within its guidelines ● Review chemotherapy order sheets for accuracy, comparing to guidelines or study protocols ● Coordinate with providers to optimize vial sizes and reduce waste ● Ensure stability of diluents and proper concentration ranges in chemotherapy orders ● Define recommended beyond-use dates and communicate with Information System Pharmacist for labeling ● Monitor lifetime doses of chemotherapy agents ● Communicate with Oncology Department providers and nursing staff regarding medication orders ● Coordinate chemotherapy prescribing plans and medication reconciliation with Oncology Department

- Chemotherapy Pharmacy (1+ years)

1+ years of experience in ANY of the following:
- EHR Coordination
- Oncology
- Doctorate degree
- Valid Pharmacist in New York (NY)
