Overview

Join our client's care team as a Home Health Physical Therapy Assistant, providing compassionate support to patients in their homes. This role offers a supportive environment with mentorship, flexible scheduling, and opportunities for career growth. You will help manage and deliver patient care plans while working closely with a team dedicated to outstanding care. Key Responsibilities ● Collaborate to provide management and delivery of patient care plans ● Support patients in Lebanon, PA and surrounding areas ● Meet critical deadlines for charting ● Self-manage time and schedule effectively Travel ● Ability to spend approximately 20% of the day driving to/from patient locations Schedule: Monday-Friday, 8am-5pm *no on-call or weekends

Qualifications

Required
  • Associate degree or higher
  • 1+ years of experience in ANY of the following:
    • Home Health Care
    • Home Health
  • 1+ years of experience in ANY of the following:
    • Electronic Medical Record (EMR)
    • Practice Partner EMR
    • EHR Implementation
  • Valid Regular Driver License (DL) in Pennsylvania (PA)
  • Valid Physical Therapy Assistant in Pennsylvania (PA)
  • Have you applied to or worked for Residential Healthcare Group anytime in the last 6 months?
  • Are you okay with the required travel for this position?
Preferred
  • Please provide your PTA license #.
